The Employee Reimbursement Form allows City employees to submit digital copies of vouchers and travel settlements.

The HPForms system is powered by Laserfiche and consists of several discrete Forms processes. Many employees already have a Laserfiche license assigned to them. However, not all employees will be able to submit using their established credentials.

For this reason, employees will generally access the form is by using a generic guest account. The password is also "guest".

Please do not sign in with the "Windows Authentication" button unless you are absolutely certain that your credentials permit submission of Reimbursement Forms.
