Before you begin, have a scanned copy of your Driver's License, any W4/NC4 withholding form needed and the following documents for each account you are enrolling:

• Voided Check, OR
• Official Direct Deposit form of your bank, OR
• A bank document that contains your name, account AND routing number on it (bank statements are not acceptable).

To create a Direct Deposit Request, or set up your NC-4 or Federal W-4 withholding allowances, you may go directly to this link to submit the official payroll form:
